- Address Vulnerabilities: Tackle the root causes of health and social vulnerabilities while strengthening community capacities and investing in preventive solutions.
- Promote Inclusion: Ensure the participation and empowerment of marginalized and vulnerable populations in community development processes.
- Combat Stigma: Reduce stigmatization and social exclusion of target groups within communities.
- Deliver Community-Based Services: Provide effective, responsive care and social protection to meet the needs of the most vulnerable.
- Build Support Networks: Establish and nurture community networks to provide sustained care and support for marginalized populations.
- Capacity Strengthening & Knowledge Sharing: Enhance skills and share learning with communities, partners, and stakeholders to foster sustainable impact.
- Tackle NCDs and Climate-Related Health Risks: Address non-communicable diseases—such as hypertension, diabetes, cardiovascular, and chronic respiratory diseases—by linking prevention and care to environmental and climate-related risks. Promote healthy lifestyles, early screening, referrals, and long-term care while reducing exposure to pollution, unsafe housing, heat stress, and other hazards, supported by integrated services at St. Martin De Porres Dispensary.
